儿童急性头痛评估

临床分类

头痛可以按病程分类

急性头痛

• 单次发作的头痛，之前未有发作。

• 可能是原发性头痛的第1次发作或不寻常的严重形式。

• 可能提示一个新的引起头痛的急性继发性原因，需要进一步评估。

急性反复发作的头痛

• 固定形式的头痛发作伴无头痛发作的间歇期。

• 大多数提示原发性头痛，尤其是这种模式的头痛已经持续了很长时间。

• 也可在继发性头痛中发生，比如伴随着间歇性颅内压升高。

慢性进展性头痛

• 头痛逐渐进展。

• 提示颅内病变在扩张。

慢性非进展性头痛

• 持续稳定的头痛。

• 可能是原发性头痛或类似的继发性病因的慢性类型。

偏头痛的诊断标准

• A：至少5次发作符合B-D标准

• B：头痛发作持续 2-72 小时（未经治疗或未成功治疗时）（注意：与成人的 4-72 小时相比较）

• C：头痛发作有下列特点中的至少2条：

  • 单侧的，也可能是双侧，额颞部*

  • 搏动性

  • 中到重度疼痛

  • 日常活动（如走路或爬楼梯）会加重头痛或头痛时避免此类运动

• D：头痛发作时至少有以下 1 个特征：

  • 恶心或呕吐

  • 畏光和畏声（可以从行为中推断）

• E:不能归因于其他疾病

有先兆偏头痛的诊断标准

• A：至少有 2 次符合 B 和 C 标准的发作

• B：存在以下一种或多种完全可逆的先兆症状**：

  • 1. 视觉

  • 2. 感觉

  • 3. 言语和/或语言

  • 4. 运动

  • 5. 脑干

  • 6. 视网膜

• C：符合以下 6 条特点中的至少 3 条：

  • 1. 至少一种先兆症状在 5 分钟期间逐渐发生

  • 2. 两种或更多先兆症状接连发生

  • 3. 每种单独的先兆症状持续 5-60 分钟

  • 4. 至少有一种先兆症状是单侧的

  • 5. 至少有一种先兆症状为阳性

  • 6. 先兆伴有头痛，或在随后的 60 分钟内发生头痛

• D：不能归因于其他疾病
